Family Circle of Care is a system of six community-based clinics providing affordable healthcare to East Texas families. As a Federally Qualified Health Center (FQHC), Family Circle of Care increases access to quality healthcare for all individuals regardless of income and insurance status. Our comprehensive medical services include family medicine, pediatrics, women’s health, internal medicine, and behavioral health.
Family Circle of Care was established in 2013 as an FQHC; however, our community care did not start there – our clinics have provided health services to families in the area since 1993.

As part of the Public Health Service Act, federally qualified health centers (FQHCs) and FQHC Look-Alikes are required to complete a Community Health Needs Assessment (CHNA) as stated under the U.S. Department of Health and Human Services Health Resources and Services Administration’s (HRSA) 19 Key Health Center Program Requirements. In 2016, a CHNA was conducted for Family Circle of Care Clinics in conjunction with CHRISTUS Trinity Mother Frances Health System and Tyler ContinueCARE hospital. The CHNA reports are a comprehensive assessment of the identified health needs in Cherokee, Smith and Wood Counties.
The analyses include a careful review of the most current health data available, demographics and input from numerous community representatives. The process culminates in the development of an Implementation Plan to address the significant needs identified through the CHNA.
In April of 2020, Family Circle of Care created a strategic plan through 2023 which includes programs that support the health of the community and the mission of the organization.
