In recognition of quality service in clinical performance, Family Circle of Care received the 2018 Quality Improvement Award from the Health Resources and Services Administration (HRSA). As a Health Center Quality Leader, Family Circle of Care achieved the best overall clinical performance among all health centers.
FAMILY Circle of Care is recognized as a Patient Centered Medical Home Level III (PCMH) by the National Committee for Quality Assurance (NCQA). PCMH is a team-based healthcare delivery model led by a health care provider that is intended to provide comprehensive and continuous medical care to patients with the goal of obtaining maximized health outcomes. Level III is currently the highest level attainable by a community health center.
Mayor Pro-Tem, Don Warren presented the proclamation on behalf of Tyler Mayor, Martin Heines, in recognition of this accomplishment. “As the most widely adopted PCMH evaluation program in the country,” NCQA Patient Centered Medical Homes deliver high quality care that can lead not only to better health outcomes, but also improved patient experiences and lower costs, the Triple Aim.
