Our team will help guide you to a plan that is right for you and affordable. One simple application will let you find out if you and your family members qualify for free or low cost coverage available through the Health Insurance Marketplace, Medicaid/Children’s Health Insurance Program (CHIP) or Sliding Fee Scale Discount Program. FCC’s Social Workers and CHWs are trained to provide guidance and support to anyone seeking assistance and this includes, but is not limited to, applying for health insurance, re-certifying for Medicaid, food stamps/cash assistance, community resources and dental care assistance, etc. We also can help with the SSI/SSDI process.
